Fixing a bug due to shitty code, shitty documentation, and shitty refactoring is a great time to fix all 3 issues (albeit in separate atomic commits) instead of being like "I left it alone to be a mystery for the next developer bahahahaha"... at least if someone is fast enough to do all 3 without dragging the entire team's productivity down.
